Travel Insurance - which company is best

I know this topic has been reviewed before, but my needs might be a little different. I am going to Japan for eight nights and staying at the Nagoya Hilton on points and if needed, I can cancel that reservation one day prior to arrival. So I only need insurance should we need to get reimbursed for airline tickets or need medical attention. I am probably looking at buying $2000 of Insurance with my biggest priority on which company works best for medical reimbursement should we need to see a doctor in Japan.

Since our main concern is for medical and evacuation, we bought an annual insurance and multiple trips coverage with GeoBlue Trekker Choice. It covers pre-existing conditions and requires the purchaser/traveller to have a primary medical insurance in the US. They have a list of doctors in each country, although we have not drilled down to check them out, where they will bill GeoBlue directly. For things like cruises, we will need to pay first and submit for reimbursement. We rely on Chase Sapphire Reserve to cover trip cost but that has a pre-existing coverage exclusion.

Btw, I was looking into the CSR travel coverage. Here is the relevant verbiage from the Guide to Benefits, page 19: "Pre-Existing Condition – illness, disease or Accidental injury of you, your Traveling Companion, your Immediate Family Member or the Immediate Family Member of the Traveling Companion, for which medical advice, diagnosis, care or treatment was recommended or received within the 60-day period immediately prior to the purchase of a Covered Trip. The taking of prescription drugs or medication for a controlled condition throughout this 60-day period will not be considered to be a treatment of illness or disease" https://www.chasebenefits.com/sapphirepreferredVW17
So the way that I am reading this is that if you are in a controlled condition and the illness has not occurred in the past 60 days before the trip is purchased, you are covered. I have additional coverage, but I have also been using the CSR for all trip purchases and expect that we will be covered one way or another. Hopefully we will not need to use it.

Yes we looked at the Chase Sapphire Reserve language on pre-existing condition and we are comfortable with it. Their medical coverage is very low and evacuation is also on the low end. Hence we decided to get a plan to enhance medical and evacuation coverage plus one that waives pre-existing condition.
