Hurray! I finally got my two rebates for phones we purchased in Oct. I called several times and even emailed them twice and had just about given up when two checks arrived in yesterday's mail. So if you haven't received yours yet, keep calling them...eventually they will send them.
 
I did not get mine, and began to worry whether I actually put it in the mailbox (I know I filled them up and put in the envolop). What is the number to call ? What info do they need ? It's been so long and I though the phone is worth it even without the rebate, but certainly better with it.
 
The customer service # is 800-784-3277 and the proceddure that you should have followed is:

Activate your phone
Mail in the completed LG5225 Mail in Rebate form and the original dated sales receipt within 14 days of purchase
Phone must be active for 60 consecutive days from date of original activation


I kept copies of all my records as they suggested and I just received 2 checks for $89 each.
