We purchased our week at the Charter Club a few years ago and attended
the owners meeting during our week. The Board mentioned that there are comment cards that can be submitted to them. I was just wondering where
I can get the comment card. Thanks for any help:)

Assuming that you want to make a comment...
You could make up one of your own. Folks call it "a letter."

My mistake......I thought that there was a specific form that could be submitted to the Board. That was the impression I got at the last meeting.

There may be a specific form but a letter will do the job just as well. So rather than looking for the form, I'd send a letter.
