I am under contract and pretty advanced (paid already) to acquire an RCI points contract. The Ebay seller is dealing through LT Transfers and this gives me a lot of confidence. I realized though that the RCI broker form they provided to me is the form the seller has sent to RCI and not the form RCI returned to them certifying the details. The form I received looks like this but it does not have the bottom part that I crossed with a red X. It also does not have the name of the person who prepared it (another clue to me it was not done by RCI). Is this sufficient? How should a dully filled out RCI broker form look like?

My first guess would be that every line should be filled in and match what you already know. My overwhelming urge is to just say that it should be boring, or at least quite dull. In any case, it should probably be filled out by the broker.

If they are using LT Transfers you can call LT Transfers yourself if you have any questions about the forms and procedure. Their number is 706-219-2709.

thanks everyone, everything is good so far, I am waiting for LT to process the deed transfer then the broker will take care of the points transfer and setting up the RCI points contract.

What I realized, I could send myself an email to brokers@rci.com. Within a day the form came back from RCI filled out with all the details and they match what I had received from the seller. I guess I was a little bit paranoid but I had not worked with this Ebay seller before and I wanted to make sure everything was OK.
In order to get the form back from RCI, you do need the names and the owners and their address (which I did)
