The first one is particularly horrific, a girl was sucked into a tube which should have been an outlet tube because a pump was installed or wired backward and nobody noticed:
‘If that pump was wired wrong, that would create the pipe was blowing water out to to suck in,' he stated.
'Right beside there, there was another pipe with a grate on it. That’s the one that’s supposed to be sucking in, because it’s basically a circulation pump.’
Inlets need a grate AND a suction break design to prevent entrapment (or worse) if the inlet is blocked.

Shocking response of hotel company to parents of girl who was sucked into pipe in lazy river

The second involves a couple who were electrocuted in a resort hot tub. The resort was located in Mexico, so IDK if there was a requirement for GFCI protection as it is here in the US.

In the first case, the hotel never had the required city inspection done before reopening the pool. The inspection should have caught the problem. No wonder the hotel is trying to stonewall the parents. I hope they pay significant damages after all is said and done.
