Paper mill in Jay Maine explosion

Just saw video of the explosion -- it was quite impressive!
 
Here is the NSFW video from one of the log trucks of the explosion. As a truck driver, I picked up paper rolls from this plant in the mid-2000s. Those who have worked in paper plants know the stuff they use to turn logs into paper is toxic, so that may be a cause. I haven't heard of deaths, but there were injuries.

I watched the local news tonight (I live in Maine), and they said NO injuries---amazing........
 
Looks like a digester blew up. The stuff that is "raining" out of the cloud and landing on the windshield looks like partially digested wood coated with digester liquor.
 
News reports today confirm it was a digester.

This requires at least two failures. First is whatever caused the digester to overpressure. How did that happen? How was it not detected by instrumentation, or if it was detected, why didn't the operators respond in time to address the situation?

Second is failure of the pressure relief system for the digester. The digester is designed with safeties to controlled pressure relief in exactly this type of situation. That system also failed.
