I am thinking of purchasing. 3br gold 5800 points with reasonable MF of $1120 for $5k asking price seems like a good deal. Is there any difference with owning outside of the US? Any hidden fees/closing costs? Any concerns?

Do you need home resort there? Otherwise you can get 7000 for $7k and MF of $800.

I don’t think I would use it immediately, just use the points. But in time I see my self going often.

If you won't be booking via Home Season at most years in Italy, I wouldn't purchase a gold season there. The MFs are too high. If you found a platinum season, then that would be a different story. But I am always thinking longer term than maybe the OP.

We own 2 platinum 2 BR weeks at Borgo. Our MFs are around $900 a week for 7000 points. Be advised that gold weeks at Borgo do not include christmas and March through September, so for those weeks you would only be able to book 9 months ahead same as all other non platinum owners.
