Reasons Why Cancel/Rebook Fails

I've read about this before, but I thought it would be a good idea to summarize the possibilites in 1 thread. I will start with things that can definitely happen.

1. After canceling, another member booked the room so you are unable to rebook.
2. After canceling, there are system/network connection problems.
3. Wyndham overbooked a unit, so after canceling, the room is never released for availability.

Now I will speculate on a couple other possibilities.
1. Wyndham takes the week for Extra Holidays to add to their profits.
2. The Wyndham rep is unethical. The rep actual stole your reservation for him/herself and told you that it was lost somehow. Are they required to certify that they are independent or can they actually be a mega-owner? I can just picture a crooked employee stealing everyone's prime week and making big bucks renting the best weeks in the system.

Have I missed any other possibilities?

And don't forget the rep that I bribe to watch for cancellations that I want and alerts me to make a move. (Hypothetical but something I've been wondering if it really happens or not.)
